Heart Failure in CKD


KDIGO is developing its first Clinical Practice Guideline for the Management of Heart Failure in Chronic Kidney Disease. This guideline will provide evidence-based recommendations for the diagnosis, evaluation, and treatment of people living with both conditions through coordinated, multidisciplinary care. The guideline is co-chaired by Nisha Bansal, MD, MAS (U.S.), and Jozine ter Maaten, MD, PhD, FESC, FHFA (The Netherlands), with an international, multidisciplinary Work Group of nephrologists and cardiologists alongside caretakers of and individuals with lived experience of CKD and heart failure.

Building on findings from the KDIGO 2024 Controversies Conference on Heart Failure in CKD, this guideline responds to the growing recognition of the close relationship between kidney and heart disease and the need for integrated, patient-centered guidance in this field. It will address shared mechanisms, diagnostic and therapeutic challenges, and approaches to optimize outcomes through multidisciplinary collaboration.

Topics will include epidemiology, pathophysiology, case-finding, diagnostic and prognostic strategies, pharmacologic and nonpharmacologic management, acute and advanced care, invasive and device-based therapies, lifestyle interventions, and optimal models of collaborative kidney–heart care. The guideline will also highlight patient-reported outcomes, considerations in special populations, shared decision-making, and barriers to implementation across care settings.
